AFFIDAVIT OF DISCREPANCY

I, JARED ANDRE MANHILOT FABIAÑA, of legal age, Filipino,

married and with a residence address at Mayor B. Atega St. POB. 10, M.I.
Compound, City of Cabadbaran, Agusan Del Norte, Philippines. after having been
duly sworn to in accordance with law, do hereby depose and state;
That my true and real name was JARED ANDRE MANHILOT
FABIAÑA, and I used the said name since my childhood day in school and all
legal documents.
That upon checking on my COP (II/4) (II/5) (SDSD) my “Last Name” was
erronously recorded as “FABIANA” which is falsed issued by MARINA MISMO.
That the name “JARED ANDRE MANHILOT FABIAÑA” is my real
name and “JARED ANDRE MANHILOT FABIANA” in my Certificate of
Proficiency, refers to me as one and the same person.
That I am executing this Affidavit of Discrepancy to the truth of the
foregoing facts and to support the replacement of my Certificate of Proficiency
and for whatever legal purpose it may do serve.
