AFFIDAVIT OF ONENESS

I, EDWINIO M. BLAS Sr., of legal age, single, and with address at Blk. 8 Lot
14B, Tulip St., Camella Home Classic Bls. Parañaque, after having been duly sworn to
in accordance with law, do hereby depose and say:

1. THAT my name is EDWINIO M. BLAS Sr. as stated in my Certificate of Live

Birth. A copy of the page of my Certificate of Live Birth where my name appears
is hereto attached as ANNEX “A”;
2. THAT the name EDWINIO M. BLAS on my PNB Card is also me. A copy of the
page of my PNB Card where my name appears as aforestated is hereto attached
as ANNEX “B”
3. THAT since childhood, my friends and acquaintances knows my full name as
EDWINIO M. BLAS Sr., and thus since then I have been accustomed to using
the EDWINIO M. BLAS Sr. in my personal records and ID’s, transactions, and
communications;
4. THAT to remove the confusion as to the names shown in the attachments, I
hereby declare that EDWINIO M. BLAS Sr. and EDWINIO M. BLAS is one
and the same person; and
5. THAT I am executing this Affidavit to attest to the truth of the foregoing facts and
to use the same for whatever legal purpose it may serve.
