AFFIDAVIT OF ONE AND THE SAME PERSON
I, MARIA JESUSA BAYAIS DOMAOAL, of legal age, Filipino citizen, married,
and a resident of 16 FALCON ST. BUNDOK HOA APITONG ST. MARIKINA
HEIGHTS MARIKINA CITY, after having been sworn in accordance with law aver that;

1. I am the child of ERNESTO D. BAYAIS and PREMIA A. FRANCISCO ;

2. I have used the name “Maria Jesusa”, in all of my records and dealings such as
government IDs, and others of similar nature.

3. My Birth Certificate and all my government IDs such as Unified Multi-Purpose

ID states my first name as “Maria Jesusa” to which my identity is based while in
my Certificate of Marriage record my name as “Ma.Jesusa” instead of “Maria
Jesusa” which is the name appearing in my Birth Certificate.

4. I am under the impression and belief that “Maria Jesusa” is my real and ONLY
name;

5. It was only during the time that I need to secure requirements for my sons’s
Certificate of Live Birth that I discovered that a name “Ma.Jesusa” appeared in
the copy of my Certificate of Marriage instead of the name “Maria Jesusa”
which I am using since childhood.

6. I am executing this affidavit to attest that “Maria Jesusa” as appearing in the

copy of my Certificate of Live Birth and “Ma.Jesusa” as appearing in the copy
of my Certificate of Marriage is ONE AND THE SAME PERSON.

7. I am executing this affidavit to attest to the truthfulness and honesty of the

foregoing facts and for all intents and purposes it may serve.

1. That we know personally FRANKLIN MIRADOR MOSELINA a.k.a.

DANILO MIRADOR MOSELINA who was born on December 27, 1961,
Filipino Citizen, of legal age and single;

2. That we know him to be using as his name DANILO MIRADOR

MOSELINA in all his private and public dealings instead of FRANKLIN
MIRADOR MOSELINA her true and correct name appearing on her
Birth Certificate;

3. That DANILO MIRADOR MOSELINA and FRANKLIN MIRADOR
