AFFIDAVIT OF LATE SUBMISSION

I, ROMMEL MADRIDANO CHAVEZ, Filipino, of legal age, and resident of

1417 Sisa Street, Sampaloc, Manila, after having been duly sworn to in
accordance with the law do hereby depose and say THAT:

1. That I am a Pensioner of AFP Pension and Gratuity Management Center

served the Armed Forces of the Philippines;

2. That I was a late in submitting the document for my Pension because I

was in Canada;

3. That I am executing this affidavit freely and voluntarily in order to

establish the truth of the foregoing facts and for whatever legal purpose
this may serve.
