CUSTODY AND GUARDIANSHIP

WHEREAS, I, REY QUINTOS CATUBAY, of legal age, Irish national and residence
of No. 3 Cluain Alainn Hurler’s Cross, Shannon Country Clare, Ireland and temporarily
residing at No. 62 Liwasang Kalayaan, Marikina Heights, Marikina City, by a certain public
instrument denominated as Affidavit of Custody and Guardianship made and executed in
Marikina City on July 11, 2018 before Atty. Anna Theresa C. Cruz, Notary Public, and
bearing document No. 307; Page No. 62; Book No. 176-B; Series of 2018 of her/his Notarial
Register, did name, constitute and appoint- MONINA M. DELGADO, as true and lawful
guardian of my father, Mario Delos Santos Catubay, for the purpose and with powers
mentioned in said public instrument;

WHEREAS, I have realized that it is to my best interest to cancel, discharge said

Affidavit of Custody and Guardianship.

NOW THEREFORE, I, REY QUINTOS CATUBAY, by virtue of these present, hereby

unconditionally REVOKE, ANNUL and make VOID the said GUARDIAN and all powers and
authority therein or thereby given or granted, or intended to be given or granted to said
MONINA M. DELGADO, effective on the date of this instrument.
