AFFIDAVIT OF DISCREPANCY

I, JUDY BILAG BELO, of legal age, Filipino, and a resident of 1213 San
Antonio St., Barangay 175, Caloocan City, after having been duly sworn to in
accordance by law hereby depose and state that;:

1. That I have used the name JUDY BILAG BELO, who was born on April 27,
1979 in Amulung, Cagayan, which appeared in all of the documents I have
signed;

2. That unfortunately in the my License to Own and Possess Firearms (LTOPF) ID

Card and Firearm Registration ID Card, my name appeared as JEDY BILAG
BELO;

3. That in view of this, I hereby attest that the name JUDY BILAG BELO and
JEDY BILAG BELO, refers to one and the same person;

4. That the inconsistency is not intended to defraud any criminal act on my part
but merely an accident;

5. That all given statement is true and correct to my knowledge;

6. That I am executing this affidavit to attest to the truth of the foregoing facts
and circumstances and to attest further for all legal intents and purposes
which may serve.
