LETTER OF AUTHORIZATION

To the authorities:

Good Day!

This authorization is a letter of request in allowing Mrs. Regina Garde to represent,myself, Mr. Arjay
Malapitan and/or Arjay Malapitan Bautista to arrange legal documents such as CORRECTION OF
NAME OF PARENTS IN BIRTH CERTIFICATE and ADMISSION OF PATERNITY in behalf of my presence. I
am not able to make it through this arrangement and can’t even take part of the said processing of
papers due to the pandemic lockdown.
Attached here below is the statement of corrections which I request to process effectively today and
onwards.

Name in the NSO: Gina Merida Malapitan (mother) -WRONG

Should be corrected to: Jinnah Merida Malapitan - CORRECT

Name in the NSO: Frederick Mendoza Bautista -WRONG

Should be corrected to: Frederick Jose Mendoza Bautista - CORRECT

To further request, if possible I would also like to fulfill my legitimacy as the son of Jinnah Merida
Malapitan (mother) and Frederick Jose Mendoza Bautista (father) be transparent and legalized in
my birth certificate. Attached in this letter are the necessary supporting documents to support this
request.

ACCEPTANCE AS AN AUTHORISED SIGNATORY

Hence, Mrs. Regina Garde, hereby solemnly accord her acceptance to act as an authorized person in
behalf of Mr. Arjay Malapitan and/or Arjay Malapitan Bautista for the above referred legal
document transactions and all my acts shall be binding on this matter.
