AFFIDAVIT OF LOSS
I, DINDO C. CABILES, of legal age, single/married, Filipino, and a resident of, #18 DON
FRANCISCO ST. VILLA BEATRIZ, OLD BALARA QUEZON CITY, after having been sworn to in
accordance with law hereby depose and say that:

1. I am a holder of GLOBE SIMCARD NO. 09958118637.

2. That sometime on June 13, 2022 at around 10:00AM, my cellphone was snatched
along MRT STATION GUADALUPE with unidentified person.

3. That immediately after discovery of such loss, I have exerted diligent effort to
recover the aforesaid missing Cellphone together with my simcard the same was
lost above stated and considered lost for all intents and purposes

4. I execute this affidavit to attest to the truthfulness of the foregoing information,

and to secure the retrieval/replacement of my GLOBE SIMCARD , and for whatever
legal purpose it may serve.
