JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ARCADIO CASERES ACQUIATAN and FELIXBERTA O. JAMERO both

of legal age, Filipino citizens, and residents of Brgy. Tabuan and Purok 2, Candanas, Garcia
Hernandez, Bohol, respectively after having been duly sworn to in accordance with law, do
hereby depose and state:

1. We personally know one LEOBERT D. BAGUHIN;

2. We also personally know one LEONARDO J. BAGUHIN JR., married to
BERNADETH C. DURANO;
3. That Leobert D. Baguhin was born on September 17 th, 2005 to spouses
LEONARDO J. BAGUHIN J.R. and BERNADETH C. DURANO;
4. That Leobert D. Baguhin has two other siblings, namely: Berjone Durano
Baguhin and Steven Durano Baguhin;
5. That we personally know Leobert D. Baguhin and his parents because our
families have long been personal acquaintances and friends;
6. That the name LEONARDO J. BAGUHIN appearing as “father” in the
Certificate of Live Birth of LEOBERT D. BAGUHIN issued by the Local Civil
Registry of Valencia, Bohol; and the name LEONARDO JR. J. BAGUHIN
appearing in the Certificate of Marriage with Registry No. 2009-28 as “husband”;
the name LEONARDO JR. J. BAGUHIN appearing as “father” in the
Certificates of Live Birth of Berjone Durano Baguhin and Steven Durano
Baguhin; refers to one and the same person, and that no other person shall answer
for such name and circumstances except LEONARDO JR. J. BAGUHIN
(Leonardo J. Baguhin, Jr.);
7. That the discrepancy in the names recorded in the aforesaid documents arose from
the filling up of personal information, particularly the “father’s name” in the
Certificate of Live Birth of Leobert D. Baguhin;
8. We are executing this affidavit to attest to the truth of the foregoing facts and for
whatever legal purpose this may serve best.
9. We are not related by affinity or consanguinity nor are we given reward to
execute this Affidavit;
10. We execute this Affidavit voluntarily and freely for whatever legal purpose this
may serve.
IN WITNESS WHEREOF, we have hereunto set our hands this 5TH day of
NOVEMEBR 2019 in Tagbilaran City, Bohol, Philippines.
