JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, MERCEDITA A. ESTOROSAS, Filipino, of legal age, married and a resident of

Tagbilaran City, Bohol, Philippines and CONRADO T. APOSTOL JR., Filipino, of legal age,
married and a resident of Cortes, Bohol, Philippines, after having been duly sworn in
accordance with law, do hereby depose and state:

1. That we personally know the late RANDY JUNIO who died on July 23, 2018 at
Purok 1 Tipolo, Ubay, Bohol, Philippines;

2. That some discrepancies appeared in the documents of RANDY JUNIO, to wit:

RANDY JUNIO AGAD- his name as reflected in his Social Security System (SSS) Record;

RANDY A. JUNIO- his name as reflected in his Certificate of Live Birth duly issued by
the Office of the Local Civil Registrar of Ubay, Bohol, Philippines;

RANDY JUNIO- his name as reflected in his Certificate of Death duly issued by the
Office of the Local Civil Registrar of Ubay, Bohol, Philippines;

3. That by virtue of these presents, we hereby ATTEST and DECLARE that the names
RANDY JUNIO AGAD per his SSS record, RANDY A. JUNIO per his Certificate of Live
Birth duly issued by the Office of the Local Civil Registrar of Ubay, Bohol,
Philippines and RANDY JUNIO per his Certificate of Death duly issued by the Office
of the Local Civil Registrar of Ubay, Bohol, Philippines, both refer to one and the
same person who is RANDY JUNIO who died on July 23, 2018, the son of Meriam
A. Junio;

4. That the discrepancies in the documents and records of the late RANDY JUNIO was
merely clerical error;

5. That we know of such facts because we are friends of the late RANDY JUNIO;

6. That we are not related to the said deceased person either by affinity or
consanguinity and that we have no interest in whatever outcome this presents may
bring;

7. That we execute this affidavit to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 7th day of August 2018 at
Tagbilaran City, Bohol, Philippines.
