URGENT MOTION TO ALLOW ACCUSED TO UNDERGO

FURTHER MEDICAL EXAMINATION

Accused JAY ANTHONY LAJOT, through the undersigned counsel, and

unto this Honorable Court most respectfully states that:

1. Herein accused JAY ANTHONY LAJOT is complaining that he is

suffering pains in the stomach and his lower back for how many days already and he is
advised for further medical evaluation and ultrasound on July 5, 2018 per referral slip
issued by the nurse on duty and duly approved by Warden J/CINSP MICHELE
POSTIGO, hereto attached as ANNEX “1”;

2. That accused having the right to health, accused humbly requests that he
be further examine by a doctor of any government hospital and be subjected to an
ultrasound;

PRAYER

WHEREFORE, premises considered, in the highest interest of justice, it is most

respectfully prayed of this Honorable Court that accused be allowed to undergo further
medical examination and ultrasound. Other reliefs deemed just and equitable under the
premises are likewise prayed for.
