Village, Brgy. Ibabang Dupay, Lucena City, after having been duly sworn to in accordance with
law hereby depose and state:

That I am the surviving wife of GUILLERMO MOSTIERO REYES,


who died on July 27, 2022 in Lucena City;

That I know that the true and correct spelling of my husband surname is
REYES, he been using since childhood and all government records and our
children Birth Certificate;

That however, I found out in our MARRIAGE CONTRACT issued by


Philippines Statistic Authority, his surname was entered as GUILLERMO
MOSTIERO DE LOS REYES;

That I hereby attest that GUILLERMO MOSTIERO REYES and


GUILLERMO MOSTIERO DE LOS REYES, refer are but one and the same
person;

That I am executing this affidavit to attest to the truth of the foregoing


facts and/or for whatever legal purposes this may serve;
