AFFIDAVIT OF HEIRSHIP AND UNDERTAKING WITH CONSENT

I, MONICA M. GABIN, of legal age, married, Filipino, and a resident of Bolocboloc,

Sibulan, Negros Oriental, after having been duly sworn to in accordance with law, hereby state
and declare that:

LIBERATO MORATO is the registered owner of a parcel of land known as Lot No.
17191-B, with an area of TWO THOUSAND ONE HUNDRED NINETY-THREE (2,193)
SQUARE METERS, more or less, situated in Bolocboloc, Sibulan, Negros Oriental, covered
under Tax Declaration of Real Property No. 2017-16-0005-01954;

That LIBERATO MORATO died intestate and left no debts or obligations due against
his estate and at the time of his death he was survived by his seven (7) children, I am one of
them;

With the death of my parents, I am entitled to inherit and/or succeed to the above-
described parcel of land through succession;

I am giving consent to my daughter MONIPERL G. ESTANIEL for her to apply for an

Electric Connection Permit on her house on the said lot. I further undertake that the
department/person/agency/ and Local government unit approving the said permit or connection
are not to be made accountable relative to its issuance and I take full responsibility in case of any
claims or complaints including the application of electric connection permit on the
abovementioned lot being one of the owners thereon;

I am executing this affidavit to attest the truth of the foregoing facts and for any other
legal purpose that this affidavit may serve;
