JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

That we, PELAGIA V. DESOR, of legal age, Filipino and a resident of Poblacion,
Sibulan, Negros Oriental, and FEDERICO A. ARANAS, of legal age, Filipino and a resident of
Magatas, Sibulan, Negros Oriental, under oath, do hereby depose and manifest that:

We personally know the person of the late “MARIA ANTONIA ABLIR REPOLLO”,
and that recently we came to know that her name as indicated in her Civil Registry Form No. IC
(Birth-Destroyed) was “MARIA CALINGACION ABLIR” and we know for a fact that the
person by the name of “MARIA CALINGACION ABLIR” appearing in her Civil Registry
Form No. IC (Birth-Destroyed) and the person with the name “MARIA ANTONIA ABLIR
REPOLLO” appearing in her Certificate of Death, REFERS TO ONE AND THE SAME
PERSON, the latter being the correct and real name;

We also personally know the person of “ARSENIO ARCHIE ABLIR REPOLLO”,

and that recently we came to know that his name as indicated in his Civil Registry Form No. 1A
(Birth Available) was “ARSENIO REPOLLO” and we know for a fact that the person by the
name of “ARSENIO REPOLLO” appearing in his Civil Registry Form No. 1A (Birth
Available) and the person with the name “ARSENIO ARCHIE ABLIR REPOLLO”
appearing in his Professional Driver’s License, REFERS TO ONE AND THE SAME
PERSON, the latter being the correct and real name;

Perhaps, the incorrect entry was due to honest mistake and inadvertence. That we know
of these facts considering that we are acquaintances of them, and that neither of us is related to
them either by affinity or consanguinity and that we are executing this joint affidavit freely and
voluntarily in order to attest to the veracity of the facts above stated and for whatever legal
purpose this may serve best;
