JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

That we, GINA BLASING GONZALVE and MARY JOY FERMIN

SANCHEZ, both of legal ages, married, Filipinos, and residents of Poblacion,
Sibulan, Negros Oriental, under oath, do hereby depose and manifest:

That we personally know the person of “ANNA MAE FONTELO

ESTORAS”, and that recently we came to know that her name and birthdate
as indicated in her SSS records/data with SS Number: 34-9650265-6 was
“ANNA MAW FONTELO ESTORAS” with a birthdate of May 11, 2020
(5/11/2020), and we know for a fact that the person by the name of “ANNA
MAW FONTELO ESTORAS” with a birthdate of May 11, 2020 (5/11/2020)
appearing in her SSS records/data with SS Number: 34-9650265-6 and the
name “ANNA MAE FONTELO ESTORAS” appearing in her Certificate of Live
Birth with a birthdate of May 11, 1998 (5/11/1998), REFERS TO ONE AND
THE SAME PERSON, the latter being the true and correct name;

Perhaps, the incorrect entry was made due to an honest mistake or

inadvertence. That we know of these facts considering that we are
acquaintances of ANNA MAE FONTELO ESTORAS, and that neither of us is
related with her either by affinity or consanguinity and that we are executing
this joint affidavit freely and voluntarily in order to attest to the veracity of the
facts above-stated and for whatever legal purposed this may serve best;
