AFFIDAVIT

I, ARGIELYN U. SPANTO,RN, of legal age, married, Filipino citizen, and

a resident of Barbers, Village, Rizal Surigao City, after having been sworn to
in accordance with law, do hereby depose and state that:

1. I am a graduate of University of San Carlos in March 2008 with a

degree in Bachelor of Science in Nursing;

2. I took and passed the Board Examination for Nursing in 2008;

3. As a nurse, I have been practicing my profession for ten (10) years;

4. I have rendered valuable service as a nurse in Miranda Family

Hospital since March 27, 2009 up to present;

5. In these fruitful years, I was assigned in the Dialyses Unit as a Nurse

Supervisor, Operating Room, Emergency Room, Delivery Room, as
well and in the clinical ward;

6. During my employment, I am working Forty-eight (48) hours per week

faithfully accomplishing the tasks assigned to me. Hereto attached is my
Certificate of Employment/Recommendation issued by Miranda
family Hospital as Annex ‘A” for your reference;

7. I am executing this affidavit in order to attest to the truth of the

foregoing statement and for any legal purpose this mu serve.
