AFFIDAVIT OF NON-RETRACTION

I,__________________, of legal age, single, Filipino Citizen and with

residential address at Kalinga, after having been duly sworn to in accordance with
law hereby depose and say:

1. That I am a member of the Armed Forces of the Philippines with a rank of

PFC and have served therein for a number of Three (3) Years and Ten (10)
months;

2. That I desire to resign from my military service effective 01 August 2021;

3. I will not retract my resignation from the military service nor work for the
revocation if my order for resignation once issued;

4. That I am executing this affidavit to support my resignation from the

military service- Armed Forces of the Philippines and for any legal
purpose it may serve.
