JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

That we, MARILYN A. TAŇAQUIN, of legal age, married, Filipino, and resident
of Balugo, Sibulan, Negros Oriental, and EMELIA G. YUNTING, of legal age, Filipino,
widow and resident of Boloc-boloc, Sibulan, Negros Oriental, under oath, do hereby
depose and manifest:

That we personally know the person of the late “LAUDIVICO DIONANAO

OMENTO”, and that recently we came to know that his name as indicated in his SSS
record was “LOUDIVICO D. OMENTO” and his name in his Marriage Contract was
“RODRIGO DIONANAO OMENTO”, and we know for a fact that the person by the
name of “LOUDIVICO D. OMENTO” appearing in his SSS record and the name
“RODRIGO DIONANAO OMENTO” appearing in his Marriage Contract, and the name
“LAUDIVICO DIONANAO OMENTO” as appearing in his Birth Certificate and Death
Certificate, REFERS TO ONE AND THE SAME PERSON, the latter being the real and
correct name;

Perhaps, the incorrect entries were due to honest mistake and inadvertence.
That we know of these facts considering that we are acquiantances of him, and that
neither of us is related with him either by affinity or consanguinity and that we are
executing this joint affidavit freely and voluntarily in order to attest to the veracity of
the facts above-stated and for whatever legal purposed this may serve best;

we know for a fact that the person by the name of “GREGORIA DIOMANO” appearing
in the Certificate of Live Birth of her son LAUDIVICO DIONANAO OMENTO, and the
name “GREGORIA DIONANAO” appearing in the Marriage Contract of her son
LAUDIVICO DIONANAO OMENTO, REFERS TO ONE AND THE SAME PERSON, the
latter being the real and correct name;
