AFFIDAVIT OF HEIRSHIP AND UNDERTAKING

I, VIRGINIA T. VALLADA, of legal age, married, Filipino, and a resident of Jilocon,

San Jose, Negros Oriental, after having been duly sworn to in accordance with law, hereby state
and declare that:

1. I am one of the surviving heirs and daughter of the late MELECIO TURTAL,
who is the lawful owner of FIVE OVER ONE HUNDRED FIFTIETH (5/150) undivided share
of a parcel of land known as Lot No. 443, situated in Sibulan, Negros Oriental, by virtue of a
document known as Extrajudicial Settlement and Sale, duly recorded and notarized as Doc. No. 80,
Page No. 31, Book No. III, Series of 1971, in the Notarial Registry of Concordio S. Ruiz ;

2. That MELECIO TURTAL died intestate and left no debts or obligations due
against his estate and at the time of his death he was survived by his six (6) children, I am one of
them;

3. With the death of my parents, I am entitled to inherit and/or succeed to the above-
described parcel of land through succession;

4. I am applying for a Building Permit and Electric Connection Permit to the Office
of the Local Building Official on the said lot. I further undertake that the
department/person/agency/ approving the said permit is not to be made accountable relative to its
issuance and I take full responsibility in case of any claims or complaints including the
application of building permit and electric connection permit on the abovementioned lot being
one of the heirs of the registered owners thereon;

5. I am executing this affidavit in order to establish all of the foregoing facts, and in
order to establish that I am one of the surviving heirs of the late MELECIO TURTAL, entitled
to inherit to the aforementioned parcel of land;
