AFFIDAVIT

That I, DEO DUDZ BAÑEZ HIZO, of legal age, single, Filipino and a
resident of Brgy. Buda, Marilog, Davao City after having been sworn to in
accordance with law, do hereby depose and state the following:

1. That since my childhood, I am habitually and is continuously using the

name DEO DUDZ BAÑEZ HIZO as my true and correct full name;

2. That my name was correctly recorded in my Driver’s License issued by the

LTO, PRC Professional Identification Card, COMELEC Voter’s ID, and Bureau
of Fire Protection (BFP) ID as DEO DUDZ BAÑEZ HIZO;

3. That inadevertently register my full name in my Baptismal Certificate and

was recorded as “DEO DODGE BAÑEZ HIZO” instead of DEO DUDZ BAÑEZ
HIZO;

4. That the person named in the Driver’s License, PRC ID, Voter’s ID, and BFP
ID as DEO DUDZ BAÑEZ HIZO and the name DIO DODGE HIZO in my
Baptismal Certificate are one and the same;

5. That I will use my true and correct name as appearing in my private and
public documents as DEO DUDZ BAÑEZ HIZO in my application for
correction of my name in my Baptismal Certificate; and

6. That this Affidavit is to attest to the truthfulness of this fact and to establish
the fact that my legal and correct name is DEO DUDZ BAÑEZ HIZO and to
have the same recorded as such by all offices concerned.
