AFFIDAVIT

I, PATRECIA GRANADA LLENAS, Filipino, married to Maceste Llenas, of legal age, with
address at La Paz II, Lapasan, Cagayan de Oro City, after having been duly sworn to in
accordance with law hereby depose and state:

1) That I am the mother of MACISTE RIZ GRANADA LLENAS;

2) That the Certificate of Live Birth issued on 30 December 2005 at Minglanilla District
Hospital, Ming. Cebu City indicates the name of my son as BABY BOY GRANADA
LLENAS;

3) That on the other hand, in the Certificate of Baptism issued on 10 August 2007
at San Roque Parish Church, Lipata, Minglanilla, Cebu 6046, Philippines the name of
my son is indicated as MACISTE RIZ G. LLENAS;

4) That both names BABY BOY GRANADA LLENAS and MACISTE RIZ GRANADA
LLENAS as evidenced by the attached documents, pertain to one and the same
person;

5) That I am executing this affidavit for the purpose of filing before the CITY CIVIL
REGISTRAR OFFICE a petition for correction of clerical error in the entry of FIRST
NAME of the Certificate of Live Birth from BABY BOY to MACISTE RIZ and to attest to
the truth of the foregoing and for whatever legal purposes it may serve.
