AFFIDAVIT
Re: Barangay Official Eligibility (BOE)

I, EDILBERTO F. CAIDIC, JR., Filipino, of legal age, and a resident of No. 1224
Dearfield Street, Brookside Hills Subdivision, Barangay San Isidro, Municipality of
Cainta, Province of Rizal, under oath, do hereby depose and state, that:

1. I was elected as a Barangay Kagawad of Barangay San Isidro, Cainta, Rizal,

and served from October 25, 2010 to November 30, 2023;

2. I am filing my application for the Grant of Eligibility pursuant to R.A.7160 or

otherwise known as Barangay Official Eligibility (BOE) at the Civil Service
Commission;

3. One of the documentary requirements before they facilitate my application is

the submission of this affidavit;

4. I hereby attest that for the duration of my term of office/service as a Barangay

Kagawad, I was neither employed in the government nor did I received any
form of salary/compensation, except honorarium for holding the position of
an ex-officio capacity during my term of office/service;

5. I declare under the penalty of perjury that the above-statements are true and
correct;

6. This AFFIDAVIT is being executed to attest to the veracity of the foregoing

statements as a documentary requirement for my application for Grant of
Eligibility (Barangay Official Eligibility) purposes not contrary to law, morals,
good custom and public policy.

IN WITNESS WHEREOF, I have hereunto set my hand this 6 th day of February

2024 in Cainta, Rizal.
