AFFIDAVIT OF DISCREPANCY

I, PRINCESS MYKA FORSUELO ABADAN, Filipino, single, and a

resident of, P-1 Barangay Port Poyohon, Butuan City, after having been duly
sworn to in accordance with law, do hereby depose and state:

1. That I was born on April 20, 1995 to the spouses LYNEAH FORSUELO
and WILLIAM ABADAN;
2. That I am the niece of MARIA CINDY FORSUELO TAGAMI;
3. That there is a DISCREPANCY in her Birth Certificate, in which her
SURNAME was erroneously entered therein as “EORSUELO”, when in
truth and in fact it’s “FORSUELO”, as evidenced by the name of his father
on the said document and by her Passport;
4. That for all intents and purposes, the name “EORSUELO” and
“FORSUELO” refer to the name of one and the same person, MARIA
CINDY FORSUELO TAGAMI;
5. That I am executing this affidavit to attest to the veracity of the foregoing for
whatever legal purpose it may serve.
