AFFIDAVIT OF DISCREPANCY

I, EUNICE JANE M. ABAO, Filipino, married, and a resident of Butuan

City, after having been duly sworn to in accordance with law, do hereby depose
and state:

1. That I am the daughter of the late TERESITA GURREA MOSQUITO,

who died on ________;
2. That there is a DISCREPANCY in her Death Certificate, in which her
MIDDLENAME was erroneously entered therein as “GOLO”, when in
truth and in fact it’s “GURREA”, as evidenced by my birth certificate;
3. That for all intents and purposes, the name “GOLO” and “GURREA” refer
to the name of one and the same person, TERESITA GURREA
MOSQUITO;
4. That I am executing this affidavit to attest to the veracity of the foregoing for
whatever legal purpose it may serve.
