AFFIDAVIT OF GUARDIANSHIP
I, , of legal age, married, Filipino citizen and a resident of P-3 Payad,
Pangantucan, Bukidnon, after being duly sworn to in accordance with law, do
hereby depose and say:

1. That I am the guardian of the deceased, who was born on June 23,
1965 at Poblacion, Kadingilan, Bukidnon and died on February 09,
2024 at Bukidnon;

2. That the above-named person is my nephew;

3. That his parents and died already;

4. That being such his guardian, I was the one providing for the basic
needs of the deceased;

5. That I have been the deceased guardian for five (5) years more or
less, that is after the death of his parents;

6. That I am executing this affidavit to apprise the authorities of the fact

of my guardianship;

7. Further, I am executing this affidavit to attest to the veracity of the

foregoing facts and for whatever legal purpose this may serve best.

IN WITNESS WHEREOF, I have hereunto affixed my hand this 13th day

of February, 2024 at Pangantucan, Bukidnon, Philippines.
