AFFIDAVIT OF PARENTAL SUPPORT AND CONSENT

We, ABC and XYZ, of legal ages, Filipino citizens, and residents of Palawan, after having been duly
sworn to according to law, hereby depose and state that:

1. We are the legitimate parents of child 1, born on 28 February 2001, who is nine (9) years of age as
of the present time and studying at Montessori.

2. Our son, Child 1, will be accompanied by and traveling with me for vacation/leisure from the
Philippines to Israel, and we will be in Israel for 2 days from March 7, 2020 up to March 9, 2020.

3. Since our son is still a minor and not yet self-sufficient, we, his parents, undertake to shoulder all
our son’s expenses during his travel abroad, including, but not limited to, his transportation expenses, hotel,
sustenance, etc. We confirm that we have the financial resources to do so because we are practicing doctors of
medicine in the Philippines.

4. I, Father, freely and voluntarily give my consent and permission to my son to travel with my wife.
Furthermore, we, parents, both guarantee that our son will not be a public charge and will comply with all the
laws, rules and regulations imposed upon transient visitors in Zimbabwe.

5. We are executing this affidavit to attest to the truth of the foregoing circumstances, to give our
parental consent and support, and for whatever legal purpose this may serve.
