AFFIDAVIT OF CONSENT FOR TRAVEL ABROAD

I, BRYAN RAYMOND S. VIAJAR, of legal age, Filipino and resident of

Block 43, Lot 34, Phase 2, Emenvile Subd., Brgy. Ambago, Butuan City,
Agusan del Norte, Philippines, after having been sworn to in accordance with
law hereby depose and say:

1. That I am the father of Rhianna Rose G. Viajar, who is currently

twelve (14) years old, and YUAN RAFAEL G. VIAJAR, who is
currently thirteen (13) years old;

2. That my children will travel with ROGELIO GUBALLA for a

tour/vacation in the United Arab of Emirates (UAE);

3. That I am giving my full consent for my children to travel with

ROGELIO GUBALLA as he committed to ensure my children’s
safety and security while with him abroad;

4. I guarantee that my children will return home after their short

vacation abroad;

5. We declare that the travel is undertaken for the above-stated

purpose and not for any purpose contrary to law, morals and public
customs;

6. Further, we execute this affidavit in order to attest to the truth of

the foregoing facts, to support our children’s application for travel
abroad from the Department of Social Welfare and Development
(DSWD), and for whatever legal purpose this affidavit may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 21 st day of
