AFFIDAVIT OF SUPPORT AND CONSENT

TO TRAVEL
I, MAMA MIA, of legal age, married, Filipino, and a resident of 1
Masuerte Street, Quezon City, after having been duly sworn to in accordance
with law do hereby depose and state:
1. That I am the biological mother of, and exercising parental authority
over minor
BABY MIA;
2. That the child will travel to Antarctica to attend the World Teen Day
2017;
3.

That the child is participating thereat as a member of the QC Teens

delegation;

4. That the mentioned child will be leaving on January 1, 2017, and will
stay in the said country until February 1, 2017, and will be under the
supervision of Mr. Papa Piccolo;
5. That I cannot
commitments;

personally

travel

with

my

child

due

to

work

6. That I am giving my full consent for the minor child to travel abroad to
be accompanied by the QC Teens 2017 delegates;
7. That I personally guarantee the financial support of the child in the
course of her travel and stay outside the Philippines and further
guarantee that she will not in any manner become public ward of any
authority of any country she may happen to pass en route to her
destination;
8. That this affidavit was executed for the purpose of attesting to the
truth of the foregoing facts and for the purpose of obtaining a visitors
visa from the Embassy of Antartica.
