AFFIDAVIT OF LATE FILING

I, RICHARD N. GONZAGA, of legal age, with residence address at G4D

Gumamela St., BNS, Ft. Bonifacio, Taguig City, after having been sworn in accordance
with law, hereby depose and state that:

1. That I am the Petitioner in the Declaration of Nullity Marriage before the

Regional Trial Court of Taguig City, Branch 69, docketed as JDRC Case No. 9064 and
wherein a Decision dated December 15, 2016 was rendered by Honorable Judge
LorifelLacapPahimna declaring the nullity of my marriage to Ella Mae V. Gonzaga;

2. That when I received a copy of the Decision granting the nullity of my

marriage, I believe in good faith that no further proceedings or procedure is necessary
to declare my marriage as null and void;

3. That my former counsel also did not inform me that I need to register the
Decision and the Finality of Judgment with the Civil Registrars of Taguig City and
Quezon City as well as with the Philippine Statistics Authority;

4. That I recently found out about the need to register the Decision and the
Finality of Judgment to complete the process of declaring my marriage as null and void
when I was applying for promotion in the Philippine Navy.

5. Hence, I am executing this Affidavit to cause the registration of the Decision

and Finality of Judgment to complete the process of the nullity of my marriage.
