AFFIDAVIT OF COMPLAINANT

I, MARGARITO LABONG y MABELANGAN, 49 years old with live-in partner a

farmer, and a resident of barangay Gap-ang Dolores, Eastern Samar, after having been sworn to
an oath in accordance with law, do hereby depose and say THAT:

1. On August 3, 2022, at about 4:00 in afternoon while passing by near the house of
Ernesto Opada when suddenly the rain pours we are invited by Ernesto to take a
shelter at their house immediately we went to their house and even he offer us some
traditional way cigarette locally known as “Mama”;

2. While I was sitting on the top of a sack of rice that has not been husked( palay )
when suddenly I noticed Bernaldo Docabo y Rabato about to hacked me with the use
of a long bladed weapon, I immediately blocked it with my left hand hitting my left
palm and my little finger was amputated;

3. I was able to hold back the long bladed weapon with my left hand supported with my
right hand while Bernaldo Docabo immediately fled away;

4. After the incident I went to Barangay to seek assistance but I was not immediately
assisted which prompt me to proceed to Casano Hospital for medication;

5. That on August 4, 2022 at about 8:30 in the morning, I went to Police station to report
the said and I was advised to go to Rural health Unit for proper medico Legal report
to support for my filling appropriate charges against Bernaldo Docabo;

I Am executing this affidavit to attest the veracity of the foregoing statements and to
support the filing of case against Bernaldo Docabo y Rabato for frustrated murder
before the Office of the Provincial Prosecutor, Eastern Samar.

IN TRUTH WHEREOF, I will affix my signature below this 22nd day of August 2022
at Dolores Municipal Police Station, Dolores, Eastern Samar, Philippines.
