I, JOHN ERIC MARTINEZ DE LA CRUZ, of legal age, Filipino Citizen,

and with residence address at B1 L37 UPECHO San Isidro, Antipolo City,
having been duly sworn in accordance with law, hereby depose and state:

(1.) That I was accustomed and known as JOHN ERIC MARTINEZ DE LA

CRUZ ever since I was a child as stated in my Certificate Live of
Birth;

(2.) That my lastname was erroneously registered in my PAG IBIG record

as JOHN ERIC MARTINEZ DELA CRUZ instead of JOHN ERIC
MARTINEZ DE LA CRUZ

(3.) That for all intents and purposes, my true and full name is JOHN
ERIC MARTINEZ DE LA CRUZ ;

(4.) That I hereby declare and attest that the foregoing discrepancies
refers to one and the same person;

(5.) That I am executing this affidavit to attest to the truth of the

aforementioned facts, and for whatever other legal purpose that it
may serve;
