AFFIDAVIT OF LATE REGISTRATION OF BUSINESS

I, RYAN BALANCIO ESCANO, of legal age, Filipino Citizen, married, and with postal
and residence address at Block 9 Lot 10 Phase 1, Fleur St., Bellefort Estates Molino IV, Bacoor
City, Cavite, after having been duly sworn to in accordance with law, hereby depose and say:

That I am the registered owner of business, REPC Computer Store - with address of
Unit D J-EE, Commercial Bldg., Lot 1, Jose Abad Santos Ave., Salawag,
Dasmarinas City, Cavite;

That the said business establishment has been registered in the Department of Trade and
Industry with business name no. 2368490;

That since I established and operated the said business on the year of 2020, I didn’t hire
any employees due to pandemic;

That during that time, I am the only one who’s managing and acting as employee;

That I just decided to hire TWO (2) employees on January 2022 and they are working
for the said business up to present;

That the said late registration was not intended, or any acts of fraudulent to commit
offence against the law;

That I hereby declare under oath that the above-statements are true and correct;

That I am executing this affidavit to attest to the truth of the foregoing facts for all legal
intents and purposes.

IN WITNESS WHEREOF, I have hereunto set my hand this 29th day of May 2023 at
