WE, MENANDRO LIMPIN LUHA and MYRA HADAIN LAKIBOL, both of legal
ages, Filipinos, and residing at Phase 5 Blk 4 Lot 16 Diamond Vill Anabu II-F Imus City Cavite
and Phase 5 Blk 4 Lot 16 Diamond Vill Anabu II-F Imus City Cavite, after having been duly
sworn to in accordance with law hereby deposes and say:

That we are family friends and neighbors of, SATRA SULAY WADJAALAM who was
born on March 13, 2004;

That when we checked the record on her Social Security System (SSS) Account we
found out that her middle name is not on the list but in her application form she written
her middle name “SULAY”

That we hereby attest and testify that his correct and true name is “SATRA SULAY
WADJAALAM”, based on our personal belief and knowledge, as well in his
identification cards;

That SATRA SULAY WADJAALAM and SATRA WADJAALAM, as described

herein, are refers to one and the same persons;

This affidavit is executed for the purpose of attesting our personal knowledge and for all
legal intents and purposes;
