AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, ROSARIO RAMOS ADAYA, resident of 240 Poblacion West Alitagtag,

Batangas, and BETTY BACONG ARAÑO, resident of San Jose, Alitagtag, Batangas
both of legal age, Filipino, after having been duly sworn to in accordance with law
hereby depose and state:

That we personally know MA. RAQUEL VERGARA ABARQUEZ because we are

longtime close friends;

That her maiden name is MA. RAQUEL GALICIA VERGARA;

That in her PSA Copy of Certificate of Live Birth (Annex “A”), her first name was
spelled and entered thereon as “MARIA RAQUEL” instead of “MA. RAQUEL” and her
date of birth was entered thereon as “June 11, 1967” instead of “May 11, 1966”;

That also in her Marriage Contract (Annex “B”), her first name was spelled and
entered thereon as MARIA RAQUEL instead of MA. RAQUEL;

That we know for a fact that her true and correct first name and date of birth
are “MA. RAQUEL” and “MAY 11, 1966”, respectively, as evidenced by her Voter’s ID
hereto attached as Annex “C”;

That the name MARIA RAQUEL GALICIA VERGARA with the birth date of
JUNE 11, 1967 as appearing on the PSA Copy of Certificate of Live Birth and Marriage
Contract and the name MA. RAQUEL VERGARA ABARQUEZ with the birth date of
MAY 11, 1966 as appearing on the Voter’s ID refers to one and the same person;

That this statement was made not to deceive any legal authority or to evade any
criminal and/or civil liability;

That we are executing this affidavit to attest to the truth of all the foregoing
facts and circumstances and to inform the proper authorities that MARIA RAQUEL
GALICIA VERGARA and MA. RAQUEL VERGARA ABARQUEZ refers to one and the
same person.
