AFFIDAVIT

I, ZENAIDA PLATINO RIANZARES, of legal age, Filipino and

presently residing at Brgy. Ambulong, Batangas City, after having
been duly sworn to in accordance with law hereby depose and state:

That I am the mother of Almario Platino Rianzares;

That on February 7, 2023, he passed away;

That since February 7, 2023, Angelito Rianzares, his father,

has not returned to our house;

That I and Angelito Rianzares are not married, attached is a

copy of the Certificate of No Marriage issued in my favor;

That I am executing this affidavit to attest to the truth of all the

foregoing facts and circumstances, and for whatever legal purpose
this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand, this 8th
