That we know a certain EMELYN AÑONUEVO CUDIAMAT who was

born on January 31, 1968 at Brgy. Talisay, Calatagan, Batangas.

That we know the facts of her life and other personal circumstances
because she is our immediate family;

That there is an error on the middle name of Father of EMELYN

AÑONUEVO CUDIAMAT as appeared on her Certificate of Live Birth
which is SIRUELOS.

That the true and correct middle name of her father is SORWELOS
as appeared on her Father Certificate of Death.

That EUGENIO SIRUELOS CUDIAMAT and EUGENIO SORWELOS

CUDIAMAT are one and the same persons.

That we are executing this Affidavit to attest to the truth of the

foregoing facts and for the purpose of rectifying the error as mentioned
above;
