Re: LATE REGISTRATION OF BIRTH

We, ILUMINADA A. BADILLO and BRICCIO A. BADILLO, both Filipino, of legal

age, and presently residing at Brgy. Cultihan, Taal, Batangas and Brgy. Carsuche, Taal,
Batangas, respectively, after having been duly sworn to in accordance with law, hereby
depose and state that:
1. We were residents of Brgy. Bolbok, Taal, Batangas since the year 1942.
2. We personally know NEMESIO C. DE SAGUN who is an applicant for the
delayed registration of his birth;
3. To the best of our knowledge, NEMESIO C. DE SAGUN was born on
November 01, 1942 at Taal, Batangas;
4. He is a Filipino citizen and a resident of Brgy. Bolbok, Taal, Batangas;
5. His parents were Lope De Sagun and Maria Consuello Cuevillas De
Sagun;
6. According to our knowledge, his birth was not registered because of distance
and constraints related to expenses at that time;
7. We are executing this affidavit for the purpose of registering the birth of the
said individual with the Philippine Statistics Authority (PSA) and for
whatever legal purpose this may serve.
