I, JE-ANN MACASADDUG TURALDE, of legal age, Filipino, a resident of Canubig 2

Calapan City Mindoro Oriental, under oath states that:

1. My name, as appearing in my UMID Card, Philippine Identification Card, TIN id, is “JE-
ANN MACASADDUG”;

2. My name, as appearing in my Philippine Statistics Authority as of February 20, 2014 is

now “JE-ANN MACASADDUG TURALDE”;

3. The names ““JE-ANN MACASADDUG” and “JE-ANN MACASADDUG TURALDE” belong to

one and the same person, the undersigned herein;

4. I am executing this affidavit in order to attest to the truth of the foregoing matters, for
setting the records straight, and for whatever legal purpose it may serve.
