AFFIDAVIT

I, FLORENCE GRACE LINDO ABACAN, of legal age, Filipino, and

a resident of Brgy. Sta. Clara, Batangas City, after having been duly
sworn to in accordance with law, do hereby depose and state:

That I am the niece of NOLAN FALTADO LINDO who recently

passed away;

That at the time of his death, he had no living spouse, children,

parents nor siblings;

That I am intending to obtain a death certificate in the name of

NOLAN FALTADO LINDO for the purpose of seeking burial assistance
before the Provincial Government of Batangas;

That I intend to obtain one (1) copy of the death certificate;

That the Office of the Civil Registrar - Batangas City has no

liability in relation to the application of the undersigned to obtain the
death certificate;

That I am executing this affidavit to attest to the truth of all the

foregoing facts, and for other legal purposes this may serve.

IN WITNESS HEREOF, I have hereunto set my hand this 6th day of
