AFFIDAVIT

I, CHRISTIAN LEONARD C. OPERIO, of legal age, Filipino citizen, married
and a resident of Sitio Kidanggin, Barangay Gandingan, Pangantucan, Bukidnon,
after having been duly sworn to in accordance with law do hereby depose and say:

1. That I am legally married to VANESSA KATE M. AMPLAYO;

2. That during our marriage, we begot a son, ACE ISAAC A. OPERIO, who was
born on October 14, 2015 at Polymendic Hospital, Valencia City, Bukidnon;

3. That I and her above-mentioned mother, who is now currently residing at

Cagayan de Oro City has been separated for more than six (6) years already,
I was left solo and alone with the responsibility of taking care and providing
support for our above-named son ;

4. That as of the moment, I am not cohabiting with any other partner;

5. That I am executing this affidavit to attest to the fact of being a solo parent
and to facilitate my request to be registered in the proper authorities as
such;

6. Further, I am executing this affidavit to attest to the

veracity of the foregoing facts and for whatever worth this may serve best.
