JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

RE: ONE AND THE SAME PERSON

WE, SAY-ON DIABE LIBERATO, married, and VIRGILIO ALABA CERVANTES,

widdow, both of legal ages, Filipinos, and are residents Barangay Hijo, Maco, Davao de
Oro, after having been sworn to in accordance with the law, hereby depose and state
that:

1. We personally known the late JULIETA LUCAS, who was our long-time
neighbor in Hijo, Maco, Davao de Oro;
2. We know for a fact that JULIETA LUCAS is the daughter of JEMENIANA
ARSOLON ALMONICAR;
3. However, there are discrepancies as to the name of her mother in her following
records:
 SSS Records GENEVIEVE / YEMENIANA
 Death certificate JERMINIANA
 Marriage Certificate JEMENIANA
4. We are not in any way related to any of the foregoing person within the fourth
civil degree either by consanguinity or affinity, nor do we have any pecuniary
interest in this matter whatsoever;
5. To remove confusion as to the identity of JULIETA LUCAS’ mother, we hereby
declare that “GENEVIEVE / YEMENIANA”, “JERMINIANA” and
“JEMENIANA” refers to one and the same person;
6. We are executing this affidavit to attest to the truthfulness of the foregoing
statements and to establish the real identity of JULIETA LUCAS’ mother
JEMENIANA ARSOLON ALMONICAR and for all legal intents and purposes it
may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 28th day of October 2022,
