WE, FREDESWINDA S. DAMILES AND ELENA A. ABAD, both of

legal ages, Filipino citizens and residents of Brgy. Ipil, Ormoc City, Leyte,
Philippines, after having been duly sworn to in accordance with law do hereby
depose and state:

1. That we personally know the late FELEXIO A. MENDOZA, because

we are longtime closed family friends and neighbors in Brgy. Ipil, Ormoc
City, Leyte;

2. That said decedent is married to Anecita Abrancia on April 13, 1981 in

Ormoc City and their marriage is duly registered with the Office of the
Local Civil Registrar, Ormoc City;

3. That however, we have come to know that in his Death Certificate

registered with the Local Civil Registrar, Ormoc City, his first name was
erroneously registered as “FLEXIO” instead of “FELEXIO” as
appearing their Marriage Contract;

4. That said discrepancy is inadvertence and it must have been caused by

erroneous entering of personal information and circumstances;

5. That the names “FELEXIO A. MENDOZA” and “FLEXIO A.

MENDOZA” refer to one and the same person;

6. That we execute this affidavit to attest to the truth and veracity of the
foregoing statements and for whatever purpose this affidavit may serve
best.

8. That however, we have come to know that in his death certificate his
name was registered as “JOSE S. KANGLEON” instead of “JOSE
FRANCISCO S. KANGLEON” as appearing in his records with
different government agencies/offices;

9. That said discrepancy is inadvertence and it must have been caused by

erroneous entering of personal information and circumstances;

10. That the names “JOSE S. KANGLEON” AND “JOSE FRANCISCO
