JOINT AFFIDAVIT OF
TWO DISINTERESTED PERSONS

We, FELIX S. GINGCO and GLORIA R. ORING, both of legal ages,

Filipinos, married, and residents of Brgy. Cagbuhangin, Ormoc City, Leyte,
after having been sworn to oath in accordance with law, hereby depose and
state that:

1. We personally know the person of OLIMPIO DELA CERNA

MAHAYAG, a resident of Brgy. Cagbuhangin, Ormoc City, Leyte,
who was born on November 17, 1960 in Libas, Merida, Leyte;

2. We know the foregoing facts because we are long-time friends and

neighbors of OLIMPIO DELA CERNA MAHAYAG and we have been
living in the same locality for a long time, and his family has been
a close family friend;

3. We know for a fact that he was born on November 15, 1960

which appears on his Certificate of Live Birth and that he has
been using the said birth date in all his documents and
transactions, instead of November 15, 1957 which appears on the
his Social Security System records;

4. We are not related in any way, either by consanguinity or affinity,

to the above mentioned person;

5. We are executing this affidavit in order to attest to the truth of the

foregoing facts in support of his application for a correction of date
of birth in his Social Security System records;

6. Further, we sayeth naught.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this
