JOINT AFFIDAVIT OF TWO DIS-INTERESTED PERSONS

We, DEXTER G. PILAPIL AND ALEJANDRINA T. LOPEZ, both of legal age,

Filipinos, and residents of Brgy. Mas-in, Ormoc City, Leyte, Philippines, after having
been duly sworn to in accordance with law, do hereby depose and state:

1. That we personally know the MARICRIS CASUQUE ROMERO-GRESOLA,

because we are longtime neighbors in Brgy. Mas-in, Ormoc City, Leyte,
Philippines;

2. That we know for a fact that said person is a member of SSS bearing SS Number
06-28618780;

3. That however, we have come to know that in her SSS record, her middle name
and inadvertently entered as “ASOQUE” instead of “CASUQUE” as appearing
in her Certificate of Live Birth registered to the Office of Civil Registrar, Ormoc
City;

4. That the discrepancies are inadvertence and it must have been caused by
erroneous typing or entering of personal information and circumstances;

5. That the true and correct name of said person is MARICRIS CASUQUE
ROMERO-GRESOLA;

6. That we can personally attest to the fact that the names MARICRIS ASOQUE
ROMERO and MARICRIS CASUQUE ROMERO-GRESOLA, refers to one
and the same person;

7. That we execute this affidavit to attest to the truth of the foregoing and for
whatever legal purpose it may serve.
