JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, BEBIANA M. RICATE and THELMA C. DICDICAN, both of legal age,

Filipinos, and both residents of Barangay Boroc, Ormoc City, Leyte, Philippines,
after having been duly sworn to in accordance with law, do hereby depose and say:

That we personally know the late ERNESTO TRANGIA CABALUNA who died
on December 08, 2020 in Ormoc City, Leyte, being legitimate child of
QUIRINO CABALUNA and EMERINCIANA TRANGIA and our longtime
neighbor and acquaintance here in Barangay Boroc, Ormoc City;

That the subject person is a bona fide member of SSS and issued a
corresponding SSS number 06-0456345-1;

That upon careful perusal of his Certificate of Death, his mother’s name was
erroneously misspelled, instead of EMERINCIANA TRANGIA appearing on
his Certificate of Marriage entered as MERINCIANA TRANGIA;

That the said discrepancy wad purely due to inadvertent herein and the said
erroneous entry is just a mere typographical error;

We are not in way related to any of the foregoing person within the
fourth civil degree either by consanguinity or affinity, nor do we have
pecuniary interest in this matter whatsoever;

We are hereby declare under oath that the name EMERINCIANA

TRANGIA per Marriage contract (mother of ERNESTO TRANGIA CABALUNA)
and MERINCIANA TRANGIA ,refer to one and the same person and that
the true and correct name of his mother is EMERINCIANA TRANGIA;

We are executing his affidavit to attest to the truth of all the foregoing facts
in order to correct the erroneous entries and for whatever lawful purpose and
intent the same may serve
