JOINT-AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, both of legal age, Filipino citizens, and residents of Brgy. , respectively, after having
been duly sworn to in accordance with law, do hereby voluntarily depose and state:

That we personally know the late the spouse of OFELIA O. MACALMA, the daughter
of and MA COP, and who was born on October 12;

That when we saw his death certificate duly issued by the Philippines Statistics
Authority, his date of birth appearing therein is October 12 instead of October 152;

That the name of his spouse appearing in his Death Certificate issued by the Philippine
Statistics Authority, because of mistake in the entry of records, is A instead of OA as appearing
in the identification cards of the latter;

That the with date of birth of Oc appearing in his Death Certificate duly issued by the
Philippines Statistics Authority, and the name ALMA with date of birth of October 12, refer to
one and the same person;

That we personally know this information because we have been friends with the late
VENANCIMA for a long period of time and maintain close acquaintance with them and their
family; and

That we are executing this affidavit to attest to the truth of the foregoing facts, particularly to
the fact that the VENAMA with date of birth of October 1952 appearing in his Death Certificate
duly issued by the Philippines Statistics Authority, and the name VQUEP MALMA with date of
birth of Octo2, refer to one and the same person;

That we are also executing this affidavit to attest to the truth of the fact that the name
OFEMA appearing in the Death Certificate of VENAN=MA issued by the Philippine Statistics
Authority, and the name OFELMA refer to one and the same person;
