ADDENDUM TO RENTAL AGREEMENT

KNOW ALL MEN BY THESE PRESENT:

This agreement made and executed this__day of _______ at Ormoc City by and between the following
parties;

BONIFACIO FESTEJO JR, of legal age, Filipino and a resident of Palompon, Leyte, Philippines,
hereinafter called the OWNER;

-and-

FNV Construction and Supply with a business address at San Roque, Isabel, Leyte represented by the
General Manager Mrs. Fe N. Valenzuela, hereinafter called the RENTER;

WITNESSETH That:

WHEREAS, On August 2, 2019, both parties contracted a Rental Agreement notarized by Atty Judd P.
Salazar of Ormoc City under Doc. No.441; Page No.87; Book No.3; Series of 2019;

NOW, THEREFORE, both parties further agree the additional terms and condition as to wit;

1. The previous contract will be extended for twenty two (22) days commencing from August 24, 2019
up to September ___, 2019;

IN WITNESS WHEREOF, the parties hereto have signed this AGREEMENT on the date and place above
written.

BEFORE ME, a Notary Public for the City of Ormoc personally appeared parties who presented to me their
respective competent evidence of identity No. – as written above; known to me and to me known to be the same
persons who executed the foregoing instrument and they acknowledge to me further that the same is their voluntary
acts and deed.

This documents consists of two (2) pages including this page where that acknowledge is written and signed
by both parties including their instrumental witnesses.
