We, CLARITA C. LUMANOG and BONIFACIO TANTANO, both of legal age,

Filipino, Resident of and with Postal Address at Barangay Bantayanon, Calatrava Negros
Occidental, Philippines under OATH DEPOSE and SAY:

1. That we personally know the person named late “EMILIO LI-OGAN SILVANO”
Residing at Barangay Bantayanon, Calatrava Negros Occidental.

2. That the name “EMILIO SILVANO” per Certificate of Baptism and “EMILIO LI-
OGAN SILVANO” per PSA Live Birth and “SILVANO EMILIO LI-OGAN” per SSS
record and “EMILIO LI-OGAN SILVANO” per Death Certificate refers one the same
person only.

3. That the name “EMILIO SILVANO” per Certificate of Baptism was born on October
10, 1956 and “EMILIO LI-OGAN SILVANO” per PSA Live Birth was born on
October 10, 1956 and “SILVANO EMILIO LI-OGAN” per SSS record was born on
October 10, 1956 “EMILIO LI-OGAN SILVANO” per Death Certificate his Date of
Birth was on September 10, 1956 was erroneously registered and encoded.

4. And said that “ROSITA C. SILVANO” the sister-in law of the Decease “EMILIO LI-
OGAN SILVANO” the informant she informed erroneously the Date of Birth of
“EMILIO LI-OGAN SILVANO” September 10, 1956.

5. That the name “EMILIO LI-OGAN SILVANO” was born on October 10, 1956 was
true and correct Date of Birth.

6. That we are executing this affidavit to establish the truth and veracity of the facts stated
above, regarding the name of “EMILIO LI-OGAN SILVANO” and the undersigned are
not related to her either by consanguinity or by affinity.

1. That we personally know the person named “MA. LORNA BELHIDA OSORNO”
residing at Brgy. Guadalupe, San Carlos City, Negros Occidental.

2. That in her marriage contract her name was entered and recorded as MA. LORNA
OSORNO and per SSS A-I of her husband late REYNALDO NORBE BEJEMINO her
name was entered and recorded as OSORNO LORNA BELHEDA as can be gleaned in
the attached document.

3. That the name “MA. LORNA BELHIDA OSORNO” per marriage contract and
OSORNO LORNA BELHEDA per SSS E-1 record of her husband refers one the same
person only.

4. That MA. LORNA BELHIDA OSORNO as her usual and customary name and the
name OSORNO LORNA BELHEDA per SSS E-1 record of her husband was
erroneously entered and encoded instead of MA. LORNA BELHIDA OSORNO.

5. That we are executing this affidavit to establish the truth and veracity of the facts stated
above, regarding the name of “MA. LORNA BELHIDA OSORNO” and the
undersigned are not related to her either by consanguinity or by affinity.

death certificate refers one the same person only.

the name NORBE ARCELE PATAGANI per SSS E-1 record of her Son and
ARACIELE PATAGANI NORBE per death certificate was erroneously entered and
encoded instead of ARACILLE PATAGANI NORBE.

5. That the name ARACILLE PATAGANI NORBE was true and correct name.
