both of legal age, Filipinos Citizens, and with residence & postal address at Blk 9 Lot 41
Karilagan St., Andreaville, Salitran IV, Dasmarinas City, Cavite and Blk 9 Lot 41 Karilagan St.,
Andreaville, Salitran, Dasmarinas City Cavite, having been duly sworn to in accordance with law
hereby depose and say;

That we family friends and neighbor of LEONARDO L. SILVA JR., who was born on
December 02, 1972 and currently residing at Blk 9 Lot 41 Karilagan St., Andreaville
Homes, Salitran IV, Dasmarinas City, Cavite;

That in his CERTIFICATE OF LIVE BIRTH, his surname appears as “SELVA” but in
his valid I.D’s, his surname is erroneously written as “SILVA”, however it is been used
as “SILVA”;

That LEONARDO L. SELVA JR., and LEONARD L. SILVA JR. as described above,
are refers to one and the same persons;

That we executing this affidavit to a[ttest to the truth of the foregoing for all legal intents
and purposes and other agencies/offices to correct/adjust her records.
