AFFIDAVIT

I, ANDREW DEL VALLE REGALADO, of legal age, Filipino,

married and a resident of Yakal St., Molave, Liboton Naga City, after
having been sworn according to law, depose and say that:

1. I was born on March 14, 1966 at Sta. Ana, Manila to Spouses

Andres C. Regalado and Lolita Del Valle.

2. I am known by the name “Andrew D. Regalado” and have used

the same in all of my personal and official records and
interactions as evidenced by my Driver’s License, Marriage
Contract and other government IDs.

3. However, in my Birth Certificate my name was erroneously

recorded as “Andres” instead of “Andrew”.

4. Likewise, my parents’ names were erroneously recorded as

follows:

Father’s Name : “Bonifacio” instead of “Andres”

Mother’s Name : “ Manuela” instead of “Lolita”

5. The names “Andres Del Valle Regalado” and “Andrew Del

Valle Regalado” refer to one and the same person, the affiant;

6. I am executing this Affidavit to attest to the truth of the

foregoing facts and for whatever purpose it may serve.
